Delayed onset muscle soreness, or DOMS, is the stiffness and ache that typically shows up a day or so after unfamiliar or intense exercise, especially exercise that involves lengthening the muscle under load, like downhill running.
In a double-blind, placebo-controlled trial of 20 well-trained men who ran downhill to induce muscle soreness, the group using a topical arnica preparation reported less pain (on muscle tenderness and a visual analogue scale) 72 hours later. Performance measures and blood markers of muscle damage and inflammation did not differ (Pumpa et al., 2014).
In plain terms: the men using arnica said their muscles hurt less three days after the run, but nothing that was measured objectively (strength, jumping, blood markers) changed. The trial was small, and it tested an arnica preparation applied every four waking hours, not a patch.
In a randomized, double-blind, placebo-controlled trial of 53 people who did calf raises, legs treated with an arnica cream reported higher pain than placebo-treated legs 24 hours later (3.04 vs 2.36 on a pain scale). Differences at days 3 and 4, muscle tenderness and ankle range of motion were not significant (Adkison et al., 2010).
None of these studies tested Arnica Patch itself. Most tested other kinds of arnica preparation, so their results should not be read as evidence about a homeopathic patch. The label lists muscle strains and overuse injuries, not DOMS by name, and it states that its claims rest on traditional homeopathic practice rather than accepted medical evidence.

DOMS usually settles by itself over several days. Stop use and ask a doctor if pain worsens, persists for more than 7 days, or comes with dark urine or severe swelling.
